Do It Online

GP Online Services

Whilst we are not currently taking appointment bookings online due to the pandemic, we are still encouraging patients to register for online services. You can order your repeat prescriptions from the comfort of home and also view your medical record (More info below). Once we resume a normal service we will reopen appointment booking online.

If you care for someone who may struggle to order prescriptions themselves you can (with their consent) apply for proxy access. This may be a friend or relative who you care for or a child aged up to 11 years old. Please ask our reception team for a form or download one here.

Access to your online medical record

To access your full medical record online please download this form and read this leaflet.

Return the completed form by email to reception.ashfield@nhs.net or post back to the surgery

A member of our team will contact and arrange to verify your ID over a video call.
